Pre-Planning Health Issues with Five Wishes® Advance Care Planning

This class explores the importance of Advance Care Planning: making thoughtful decisions before a medical emergency, serious illness, or terminal diagnosis arises. Together, we will discuss the benefits of planning ahead and work through the Five Wishes® Advance Care Planning form step by step. Even if you choose not to use the form itself, it will provide a valuable framework for considering the many “loose ends” that deserve attention: your preferences for medical treatment, your definition of dignity, how you would like to be cared for, and how you wish to be remembered as life draws to a close.

Each participant will receive a Five Wishes® booklet to take home, offering guidance as you reflect on your values and wishes. This process empowers you to ensure your voice is heard and your choices are respected-even if you are unable to communicate them yourself.

Julie Barnes Weaver

Julie Barnes Weaver is an End of Life doula, trained through INELDA (International End of Life Doula Association), a Hospice Volunteer trained by Hennepin Healthcare (formerly HCMC). She is also a Healing Touch Practitioner, a Reiki master practitioner, and a Certified Facilitator for Five Wishes Advanced Care Planning. Julie has vigiled with the dying and considers herself a shomer, one who will stay with an individual who has died to honor their death, until the funeral home arrives to transport the body.
