Planning for your future is very important, especially when it comes to your health and wellbeing for yourself and your loved ones. Come join us to learn what to know about Advanced Care Planning, and what you can do to start documenting this process and making your wishes known. We will also talk briefly about what Palliative Care and Hospice provide and requirements for services. We will have presenters on hand after the presentation with materials for you to start filling out a MN Health Care Directive and to answer any questions you may have about the forms or the process.
Sarah McDunn has been a nurse since 2002 working inpatient for half that time in oncology, and then working in outpatient hospice. Sarah has been working in an educator role since April 2023 with Park Nicollet Hospice and has a passion for patient education.
Luann Lindell Paulson is a social worker who has also worked within Park Nicollet both in the hospital and for outpatient hospice.
Michelle Cotter is an Advanced Care Planning Coordinator for Park Nicollet as well.
We all have a passion for community education and brining awareness on the importance of Advanced Care Planning among other topics into the community.
